Prevention workstream
The Prevention workstream works with stakeholders across the East of England to address current population health challenges and identify realistic solutions for those delivering healthcare.
What will the prevention workstream do?
- We bring practitioners, academics and other system partners together to identify and support projects that can drive evidence-informed, relevant and sustainable change locally.
- We apply systems approaches to include diverse perspectives and to understand challenges, priorities and potential solutions at local and regional level.
- We are adopting a task force approach to bring practice-based and academic experience and expertise to help steer and advise on each project.
- We support Fellows to address current challenges and generate learning that can be applied across the wider system. This includes providing protected time, bringing relevant partners together, facilitating and funding patient and public involvement, and offering mentoring and training to build skills and capacity.
Prevention workstream projects
We are delivering four projects across the following three themes:
Theme 1. Building Inclusive Prevention
- Exploring inclusive and equitable decision-making within a CVD prevention pathway: Adopting a systems lens to map decision-making processes within CVD prevention pathways and exploring opportunities for including patient and public voices.
Theme 2. Developing Prevention‑Ready Places
- Health Check delivery models, reach and impacts: Working with the Regional Public Health team to explore different delivery models for health checks, and how these may influence reach, access, uptake, and health inequalities.
- Developing a co‑produced, place‑based service map to navigate children and young people’s mental health support: Helping professionals, families, and communities to navigate support, while strengthening shared understanding and accountability across the system. (HFE Visiting Fellow Project)
Theme 3. Making Prevention Work
- Exploring how Warm Homes and wider prevention initiatives can be embedded into commissioning, metrics, and cross‑sector delivery: Aligning environmental, health, and social care priorities to reduce demand and improve outcomes at scale. (HFE Visiting Fellow Project)
